I figured it out! About halfway through I said "I bet neither of them are real." (As [livejournal.com profile] jblum says it was obvious Upper Leadworth wasn't real).

The story was pretty cool; a choice between two realities, which is actually a choice between the Doctor and Rory. And then it gets spun round so choosing "the Doctor's reality" is choosing Rory. And the Dream Lord, well, he's the Valeyard done right.

If there's a criticism to be made it's this: What's with the anti-ponytail sentiment?

But that's just it - the next time someone wants to write about the embodiment of the Doctor's dark side, they won't feel obliged to use a dull continuity nightmare of a character with a ludicrously complex plan to seize control of the Doctor's regenerations and take over Gallifrey. They can use a character who gets some good lines, has a relatively simple origin, and simply wants the Doctor to suffer and die.
